hehehe. don't you love those? I spent two hours (TWO LONG HOURS) hearing this lady yammer on about kidney problems and her men friends then went to check up her file to find out she was admitted for lower back pain to do with her vertebrae. hrrrrrgh.

In fact you ask...

Do you have any past medical problems?
Do you have any illnesses?
Have you ever been diagnosed with a disease?
Do you take medication for anything?
Have you had any past surgeries?
Have you been in hospital in the past?

Also
Are you allergic to anything?
and
Have any medications given you a rash or other side effects?
are two completely different questions! :P

to the same patient because the questions always bring up different answers... *sigh*

I now get their history and then continue to ask if they have diabetes, hypertension and hypercholesterolaemia, because to them, it's not an illness or disease.

You get used to it after a while...

And just like "a patient is lying until proven innocent" rule,
any female 15-45 is a complete slut and is pregnant until proven virginial.

god that's a good list, nat. what i get REALLY FREAKING ANNOYED bout is when patients start getting annoyed at you asking all these questions, insisting that 'i haven't got a past medical history, don't you understand' and then you ask if they have high blood pressure and they say 'yes but that's all' and then you ask if they have asthma and they say 'yes' and then you ask if they have high cholesterol and they say 'well that's not a disease, is it' and you think WHATEVER, HAVE FUN LIVING IN YOUR DREAM WORLD!
